2. Registration conditions:

Agency product qualification: provide "Medical Device Manufacturing Enterprise License" (domestic) or general agent company medical device business license (import), manufacturer or general agent company’s "Power of Attorney" or "Power of Attorney", medical device Product registration certificate, manufacturing approval form/registration form and attached pages.

4. The copies of the above certificates must be stamped with the company's red seal and printed in order and bound into a book.
5. Quotation requirements: Quotation according to the procurement items. The content includes the serial number, product name, model specification, manufacturer, unit, bid quotation, and market reference price (provide the use of hospitals in Guangdong Province and related bid or supply prices).
6. Operating company address, telephone number, fax number, bank account and account number, etc.
7. The registration unit shall be responsible for the authenticity of the information provided. If any fraud is found, it will be immediately disqualified and included in the list of bad behavior records. It will be prohibited from participating in the competition for the procurement of medical consumables in our hospital for three years.
3. Material collection time: from 9:00 a.m. on August 21, 2012 to 11:30 a.m. on August 25, 2012. Late arrivals will not be accepted.
